What happens during breast augmentation

Breast augmentation occurs under general anesthesia and takes around an hour and a half, depending on the placement method. There are different ways of inserting the implants into the breast:

Through the inframammary fold

When the implants are inserted through the inframammary fold, the incisions are made on the underside of your breasts. The advantage of this method is that the surgeon can place larger implants and has a good view of the chest cavity. The risk of post op bleeding and other complications is relatively small with this procedure. Although the scar is quite large compared to other possibilities, it is well hidden in the inframammary fold and barely visible after a year.

Through the armpit

In this procedure, the incisions are made in the armpit. The great advantage of this method is that there are no scars on or under the breast, but there is a greater chance that the implants shift. The surgeon also has a more limited view of the chest cavity using this procedure.

Through the areola

The incisions in this method, are made at the transition from the areola to the breast. The scar is small and inconspicuous. This technique is particularly suitable for the insertion of smaller implants because only a narrow incision is made. This procedure is often combined with a small breast lift. There is however a risk of sensory disorder in the nipple with this method.

Implants placement

The implants can be placed in front of the muscle, behind the muscle or partially in front and behind the muscle. The surgeon will choose the best method for your breast augmentation depending on your skin condition, the amount of skin tissue in your breast, the type of implant and the result you desire.

Implants are usually placed above the pectoral muscle when:

  • Your breast contains enough tissue of its own to cover the implant.
  • Your pectoral muscles are very strong, strong pectoral muscles can cause displacement of the implants.
  • You wish for a big cup size.

The downside of placing implants above the pectoral muscle is that the breasts can hang sooner than with other methods.

In most cases, the implants are placed partially above and below the pectoral muscle (dual plane). One of the advantages of this placement method is that there is less risk of capsule formation in the breast. The result also often feels and looks more natural. The downside of this method is a longer recovery period and more pain during the first weeks after surgery.

What is GalaFLEX ®?

GalaFLEX® is a temporary, absorbable scafold used to strengthen, stabilize and support weakened soft tissue. It was designed to provide support immediately after breast surgery, which allows your body to heal and create a healthy new tissue plane of primarily mature collagen.

GalaFLEX® scafolds are made from a biologically derived polymer known as poly-4-hydroxybutyrate (P4HB™), which breaks downover time into CO2 and H2O, a process similar to absorbable stitches. This means that your body knows how to metabolize and completely absorb the scafold safely and naturally.

GalaFLEX® scafolds are offered in a variety of shapes for use in breast surgery procedures. Each GalaFLEX® scafold is selected by your surgeon specific to your body’s shape and needs.

How does GalaFlex work?
During surgery, your surgeon will place the GalaFLEX® scafold internal bra under your tissue.
The unique knit pattern of the scafold encourages rapid cellular infiltration and the formation of new collagen which is a key component of healthy skin tissue. This newly formed tissue is pliable yet provides strength, support and stability to the elevated tissue.
After 18-24 months, the scafold is essentially completely absorbed and eliminated from the body as water and carbon dioxide through natural physiologic pathways. No polymer metabolites remain after the degradation process is complete. The collagen tissue that remains is 3-4 times the original tissue strength.

The GalaFLEX® Family of Scafolds are used in patients who need additional support because of weakened or deficient soft tissue, in breast surgery procedures. The GalaFLEX® Family of Scafolds are used in breast procedures such as reduction mammoplasty, augmentation and revision surgeries.

GalaFLEX® Scafold Integration

7 Months – Mature collagen is prevalent and provides reinforcement to the elevated tissue.

The Issue With Tissue
As we age so does our skin. After the age of 20, our skin loses about 1% of its collagen each year. This gradual loss of collagen is what causes skin to become thinner and less elastic, resulting in natural sagging and wrinkles.

There are many factors that can accelerate this effect, including:

Menopause (Causing an annual decrease of over 1% of skin thickness and 2% of collagen)
Previous surgery (Scars only heal to 80% of their initial strength)

In most Breast surgery procedures, compromised and/or weakened soft tissue is tightened. Unfortunately, the tissue itself will not be stronger and gravity may once again cause tissue to droop or sag. Intended Use

Breast augmentation results

Breast augmentation is a reliable and safe procedure. The result is immediately visible after the procedure. But keep in mind that your breasts will be wrapped in compressive bandages after the surgery, so you won’t be able to see your new breasts immediately. Your breasts will also be swollen at first. The end result is visible around 6 months later. The scars will have discolored after a year and will barely be noticeable anymore.

Recovery after breast augmentation

Your body needs to recover from the surgery just like after any surgery. Our care team will ensure the healing goes as smoothly as possible. After the breast augmentation, your breasts will be covered by a support bandage. You will wear this bandage for the first 48 hours. Thereafter you will wear a special postoperative bra. This bra looks like a sports bra and gives your breasts the support they need. Your doctor will advise you on how long you need to wear this bra. You may also need to wear a breast strap to lower the implants faster.

Your breasts can feel painful at first. Most patients experience a feeling of pressure on the chest. The pain is in most cases not serious and can be well controlled with medication. In a few cases, drains also need to be inserted to drain wound fluid.

We advise you not to lift your arms higher than shoulder level for the first 6 weeks after surgery. It is also wise not to lift or exercise. After 6 weeks, you can start moving normally again. Make sure however that your breasts remain well supported. We also advise you not to wear underwired bras in the first year.

Safety through precautionary measures

Once you have decided to undergo breast augmentation, we will provide you with a patient information document, which, in addition to general advice, will provide you with advice that is specific to you and based on your health and living conditions.

Before the surgery

  • Stop smoking four weeks before surgery and do not start again for at least 2 weeks afterwards. Smoking slows wound healing and increases the risk of infection.
  • Should you be taking Marcoumar, Sintrom/Sintrommitis, Asprocardio or similar medication, discuss with your surgeon whether you can temporarily stop using them. This also applies to Aspirin and painkillers with aspirin such as Advil, Diclofenac, etc. During the period around the surgery, you can use painkillers such as Paracetamol and Panadol.
  • Do not take blood thinning medication before surgery.
  • Do not forget to arrange your transport home. You can not drive yourself after the surgery and it is forbidden.
  • Ensure you are not alone for the first few nights after the surgery and that someone is available to help you in your home for the first few days.
  • Don’t forget to give us your measurements and details for the postoperative bra. We recommend that you buy two bras, this is for your own comfort and hygiene.

The day of the surgery:

  • Remove all jewelry, watches and piercings. The clinic is not liable for loss and / or theft.
  • Do not eat or drink anything for the 8 hours before the surgery.
  • Let your “driver” know at what time you can be picked up. The doctor will give you an estimate. You will remain in general at the hospital for a few hours after the surgery and in some cases it is advisable to stay the night.

After surgery

  • You will be supervised by the nurse, the anesthetist and the plastic surgeon until you leave the clinic or hospital.
  • We give most patients, in consultation with the surgeon, a prescription for medication (pain relief).
  • The breasts will feel somewhat tense and swollen after the surgery. The skin has to adapt to the new breast volume. The tension decreases by itself.
  • Only sleep on your back for the first period. After two weeks you will be able to sleep carefully on your side.
  • Rest well for the days after surgery.
  • Discuss with your doctor, when you can shower again.
  • Don’t lift your arms above your shoulders for the first two weeks. Don’t lift heavy things.
  • Keep in mind that you will not be able to work in the first week after the surgery. If you have physically strenuous work, it may be longer before you can resume work. Always consult your doctor first.
  • Sports however are not recommended for the first 6 weeks after surgery.
  • Swimming or sauna are not recommended for the first 6 weeks.

Risks and complications

Every surgical procedure carries a risk of complications. Here are some of the risks and possible complications of breast augmentation:

  • Unsightly scarring
  • Internal or external haemorrhage (haematoma)
  • Infection
  • Poor wound healing
  • Changes in (the feeling of) the nipple. This can be temporary or permanent.
  • Wrinkling of the skin over the implant
  • Double bubble: this causes the implant to sag under the skin, creating a double breast.
  • Moisture build-up or capsule formation, which makes the breasts feel hard
  • Sore breasts

The personal consultation

The first step towards more beautiful breasts is a consultation with our plastic surgeon. This conversation is of course, entirely non-binding. Each body is different which is why it is important for the surgeon to be able to look at you personally. Take this into account and wear comfortable clothing.

You choose, together with the surgeon the implants that best suit your body and wishes.

The surgeon will also ask you about your health and medical history. Give as accurate information as possible, this is important for your own safety. The surgeon will tell you, in all honesty if he thinks it is wise to perform the desired procedure and will go through it with you. If a breast augmentation with implants is not right for you, he will mention other solutions. Take the opportunity to mention other treatments or wishes you may have.
